In addition, as a Florida SHOTS certified provider, you will be:

Participation Requirements

To become a Florida SHOTS certified software provider, you must have at least one Florida client in any of these specialties:

  • Pediatricians
  • Family practitioners
  • Internal medicine physicians who immunize children
  • Urgent care clinics
  • Walk-in clinics
  • Immunologists who immunize children
  • In-hospital providers who request immunizations for newborns or other children
  • OB/GYN
  • Emergency room physicians
  • Military base physicians
  • Any other providers who administer vaccines
  • Juvenile detention centers/prisons

Immunization data can be submitted in one of these file formats:

  • HL7
  • Pipe-delimited
  • Comma-delimited
  • Fixed-length
